Steuben Rural Health Network: Work Plan ’10 Prep. – Health Activity Assessments

Overview:  The Steuben Rural Health Network (SRHN) is an organization of health and human service providers dedicated to improving community health and wellness in Steuben County. Through collaborative committees and work groups, the SRHN assists member agencies in performing health and wellness activities, and offers capacity-building services that assist the member agencies themselves.

SRHN activities are guided by an annual Work Plan. Midway through each Work Plan year, the SRHN invites committee and Work Group members to help outline the goals, objectives, activities, and strategies that the Network will focus upon through the course of the next Work Plan year. Please help the SRHN organize subjects to consider at our Annual Meeting by providing answers to the following.
